In order to best serve you, please review the policies and financing information on this page. There are two types of accounts at Western Costume: an in-house account and a credit card account. The applications for both types are available below. For an in-house account fill out the Credit Application Form, and for a credit card account please fill out the Credit Card Authorization Form. We do ask that you allow us 5 to 7 business days to process the in-house application once it is received.
